Coordinates, negotiates, procures and manages care of patients to facilitate cost effective care and patient satisfaction. Facilitates the continuum of care, works collaboratively with interdisciplinary staff, internal and external to the organization.

Reliable mode of transportation needed for travel within the region.

Qualifications

EDUCATION: Bachelor's degree in Social Work.

EXPERIENCE: 1 year recent case management experience; 3 years diverse clinical experience.

LICENSE/CERT/REG: Valid Social Worker license in the State of Maryland. LICSW preferred.

SKILLS: Ability to use computer to enter and retrieve data. Knowledge of Microsoft Office. Excellent communication skills.
